The initials "T.D." stand for Thirumala Devaswom (Sanskrit for "belonging to the Lord of Thirumala"), reflecting the institution's origins under the patronage of the T.D. Temple at Anantha Narayana Puram in Alappuzha.

Established in 1963, the institution became the fourth government medical college in Kerala after the district headquarters hospital at Alappuzha was upgraded and transferred to it for use as a teaching hospital.
